In you are attending a retreat in-person, please fill out the next section so that we can accommodate your diet, roommate requests, medical needs, etc. If you are attending online only, you may skip this section.

I understand Dewachen is a masterpiece in progress in the middle of the jungle in Costa Rica. I understand it is common in rural situations for electric or water to go out temporarily. I understand bugs and critters co-exist in the jungle. Though preventative measures are taken by the hosts, it is possible I may see them in and around personal or shared spaces. (Our service team will happily help you.)	I understand and accept. I have questions and need somone to contact me.

Diet Notes	Do you have any food allergies or dietary restrictions? If so, please indicate what foods you cannot eat and severity (for example: are you just Gluten Free vs Celiac?)
Are you taking any medications?	Please list ALL medications you are taking, we need to be aware in the unlikely case of an emergency so we can be mindful of contraindications. If you are attending an online retreat, please write N/A.
Do you have any allergies to specific medications, insects, or other? We need to know things like this in the unlikely case of an emergency.	(For example: I am allergic to bees and I carry and EpiPen. Or, I am allergic to sulfa meds.)
Do you need a chair for the meditation room? (As opposed to a floor pilllow.)	Yes No
We encourage participants to do 45 minutes of Karma Yoga, or volunteering, per day. This allows participants to contribute to the retreat environment while practicing mindfulness and selflessness in their actions. If you are interested in participating, please inform us of your preferences for volunteering tasks by choosing one or several of the tasks listed.	Taking care of the altar Ringing the gong 10 minutes before each session Sweeping the medidation deck Bringing tea to Lama Arranging meditation cushions at the end of each day Restocking the water stations Tyding the shoe area and entrance Miscellaneous kitchen tasks (example: putting away dry plates and cups) Taking out the compost Sweeping the kitchen and dining area Tidying the showers and bathrooms I would be happy to assist in any task that is required I am not interested in Karma Yoga / I am attending online only.
How do you plan to get to Dewachen?	I am bringing my 4x4 vehicle, and I DO have space for others I am bringing my 4x4 vehicle, and I do NOT have space for others I would like to share a taxi with others I would like to rent a 4x4 vehicle with others Other 4x4 vehicles are needed to get up to Dewachen. We want to encourage carpooling in vehicles and we especially want to encourage people bringing shared vehicles on site because there is a steep incline between the meditation area and the cabins. You might prefer to drive/ride up each day.
